METAL/NONMETAL
MINE FATALITY - On November 10, 2006, a 41
year-old equipment operator, with 4 years mining experience was fatally
injured at a sand and gravel operation. The victim and a co-worker were
standing underneath the head pulley section of a conveyor preparing to
attach a chain that was to be used to move the conveyor. The bolts
connecting the conveyor truss sections in the conveyor's frame failed,
causing the head pulley section to fall and strike the victim.

METAL/NONMETAL MINE FATALITY - On
October 12, 2006, a 24-year old maintenance coordinator, with 1 year and 4
months mining experience, was fatally injured at an iron ore operation. The
victim was troubleshooting an electrical fault in a high-voltage motor
control center when a sustained arc blast occurred.

October 10, 2006, a 45-year old dredge operator, with 9 years experience,
was fatally injured at a sand and gravel operation. The victim drowned when
the floating clamshell dredge he was operating capsized.

METAL/NONMETAL MINE FATALITY - On
October 4, 2006, a 59-year old contractor dragline operator, with 12 years
experience, was fatally injured at a sand and gravel operation. The victim
and other employees were preparing to repair the transmission on a
dragline. While the cab was being removed, it tipped and pinned him between
the cab and the handrail on the walkway.

METAL/NONMETAL MINE FATALITY - On
August 29, 2006, a 61-year old contractor paving supervisor, with 31 years
experience, was fatally injured at a crushed stone operation. The victim
was walking from the mine's main shop toward the plant when he was struck
by a front-end loader. The bucket was loaded with rock and was positioned
several feet off the ground, blocking the operator's view of his
surroundings and the victim. The crew was preparing to pave an area behind
the main shop.

METAL/NONMETAL MINE FATALITY -
On August 17, 2006, a 37-year old contractor employee, with 15 years
experience, fell through an access opening near the discharge end of a kiln
and landed on the concrete floor 28 feet below. The victim was attempting
to move a sheet of plywood that covered the opening when the accident
occurred. He died on August 21, 2006, as a result of his injuries.

METAL/NONMETAL MINE FATALITY -
On July 27, 2006, a 25-year-old contractor laborer, with 2 years
experience, was fatally injured at a crushed stone operation. The victim
was using a gasoline-powered weed trimmer, equipped with a circular steel
blade, to cut weeds and brush near a power pole when he struck the guy wire
for the pole. The blade severed the guy wire, causing it to contact the
energized supply conductors on the pole mounted transformer and
electrocuted him.

On July 15, 2006, a
61-year-old truck driver, with 40 years experience, was fatally injured
at a sand and gravel operation. The victim was in the pit area to get his
first load of the shift and backed the truck into a water filled pit. The
area had been flooded due to recent rains and was idle for several days
making it difficult to identify the edge of the pit.

June 28, 2006, a 49-year-old utility man, with 20 years experience, was
fatally injured at an iron ore mine. The victim was washing the roof over a
kiln when he fell through a hatch cover.

MINE FATALITY - On June 21, 2006, a
75-year-old contractor electrician, with 50 years experience, was fatally
injured at a sand and gravel operation. The victim contacted energized
conductors while installing an electrical circuit. The building
subsequently caught fire and burned before the primary circuit could be
deenergized.

METAL/NONMETAL MINE FATALITY - On
June 12, 2006, a 39-year-old plant operator, with 14 months experience, was
fatally injured at a crushed stone operation. The victim was starting to
repair a hydraulic line on a front-end loader that was parked outside of
the shop. He was underneath the raised boom, loosening a hydraulic
connection, when the boom arms fell, pinning him against the frame.
